Job Summary
Provide In-Home Health Evaluations as a Part-Time Clinician (Nurse Practitioner or Physician Assistant) delivering visits of about 45 minutes in the members’ homes. Activities include a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests (e.g., spirometry, bone density screening, diabetic eye exams) as ordered by the health plan. Use an iPad and an intuitive clinical workflow; not required to prescribe medications, order labs, or alter current treatment plans during visits. Responsibilities include visiting millions of members nationwide to connect them with the right care, traveling locally within licensed states, and occasionally conducting virtual visits. Primary workplace is the member’s home, which may include pets or allergens; flexible to travel and work within the described environments.

- Board certification (required)
- Multi-state licenses (preferred)

- Part-time role without prescribing requirement during home visits
