Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to perform services such as bone density screening and spirometry for health plan members. Engage in one-on-one conversations to answer health questions and provide education without prescribing medicine or altering treatment regimens. Travel locally and conduct virtual visits as needed while maintaining safety in diverse home environments.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
