Job Summary
In-Home Nurse Practitioner or Physician Assistant for Signify Health (CVS Health) performing 45-minute in-home health evaluations. Clinicians will conduct medication and medical history reviews, physical evaluations, and, if ordered by health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams). The role uses an iPad and standardized clinical workflow; prescribing or altering current treatment is not required. Full-time position with travel to member homes within licensed states. Responsibilities include building trusted relationships, visiting members in their homes, and delivering flexible, comprehensive, and personalized health evaluations to bridge care gaps. Qualifications include active, unrestricted NP/PA licenses, board certification, and multi-state licenses (preferred). - Ability to travel locally within licensed states
- Experience with Diagnostic & Preventive Services (e.g., spirometry, bone density screening)
- Clinical experience as NP or PA in relevant settings
