Job Summary
Conduct 45-minute in-home health evaluations including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Use an iPad and clinical workflow to connect one-on-one with patients, review medical history, and provide preventive services such as bone density screening. Perform virtual visits as needed and travel locally within licensed states to visit millions of people nationwide. Work in diverse home environments that may contain pets or allergens while fulfilling requirements as a Signify Health clinician without prescribing or altering treatment regimens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Anticipated Weekly Hours 40
- Time Type Full time
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
