Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical examinations, and additional diagnostic tests such as spirometry and diabetic eye exams when ordered. Utilize an iPad and clinical workflow to perform these services, which may include bone density screening and other preventive care products. Engage one-on-one with patients to answer health questions and provide education, while remaining flexible to travel locally across licensed states and conduct virtual visits as needed. Perform duties safely in diverse home environments that may contain pets, secondhand smoke, or airborne allergens. Work full-time hours covering the specified primary locations with a base salary ranging from $87,035 to $187,460.

- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Anticipated Weekly Hours 40
- Time Type Full time
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
