Job Summary
In-Home Nurse Practitioner or Physician Assistant to provide in-home health evaluations (approx. 45 minutes) for Signify Health as part of CVS Health. Conduct medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, DEE, bone density screening). Use an iPad and clinical workflow; not required to prescribe medications, order labs, or alter current treatment plans. Travel locally to visit members in their homes and deliver flexible, comprehensive, personalized health evaluations. Primary workplace is the private residence of health plan members, with environments that may include pets or smoke. Full-time with benefits including health/dental/vision, PTO, 401K, CEU allowance, malpractice coverage. Hours anticipated at 40 per week with a pay range of $78,331.00 - $168,714.00; actual offer determined by experience, education, geography, and other factors.
