Job Summary
In-Home Nurse Practitioner or Physician Assistant needed to perform in-home health evaluations for health plan members. The clinician visits members in their homes for ~45 minutes, reviews medications and medical history, conducts a physical evaluation, and may order additional diagnostic tests (e.g., spirometry, diabetic eye exams) as ordered by the health plan. Uses an iPad and a clinical workflow; not required to prescribe medications or alter treatment. Requires active NP/PA license(s) with board certification; ability to travel locally within licensed states; virtual visits as needed; comprehensive benefits package offered.
