Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams, using an iPad-based clinical workflow. Engage one-on-one with patients to answer health questions and provide education, while reviewing medical history and performing preventive screenings. Perform virtual visits as needed and travel locally within licensed states to reach clients in their private residences. Maintain active, unrestricted licenses and board certification to fulfill role requirements while supporting Signify Health's mission to simplify care nationwide.
