Job Summary
In-Home Nurse Practitioner or Physician Assistant needed to perform in-home health evaluations for a national clinical network. Responsibilities include conducting 45-minute visits with medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by the health plan. Use an iPad and a clinical workflow; no obligation to prescribe medications, order labs, or alter current treatments. Travel locally within licensed states; virtual visits as needed. Primary workplace is the patient’s private residence with potential exposure to household pets and other environments; flexibility to visit members in their homes and provide diagnostic and preventive services (e.g., spirometry, bone density screening) as indicated.
