Job Summary
In-Home Nurse Practitioner or Physician Assistant: provide in-home health evaluations to health plan members, performing med history review, physical evaluation, and, when ordered by the health plan, additional diagnostic tests (e.g., spirometry, bone density screening). Conduct visits using an iPad with a streamlined clinical workflow; travel locally within licensed states as needed and conduct virtual visits as needed. Join Signify Health as a clinician within a national network dedicated to improving health outcomes; primary workplace is the member's home.
