Conduct 45-minute in-home health evaluations, including medication reviews, physical exams, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad and clinical workflow to provide personalized care while connecting with patients in their private residences. Perform virtual visits as needed and travel locally within licensed states to reach members nationwide. Deliver all Signify Health services, including preventive screenings, without prescribing medication or altering treatment regimens. Maintain comfort and safety in diverse home environments that may contain pets or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
