Job Summary
Provide in-home health evaluations as a Nurse Practitioner or Physician Assistant to health-plan members in their homes. Conduct 45-minute visits including medication/medical history review, physical evaluation, and optional diagnostic tests ordered by the member’s health plan. Utilize an iPad and clinical workflow; no prescribing, ordering labs, or altering current regimens required during visits. Travel locally to member residences; primary workplace is the member’s home. Collaborate as part of Signify Health/CVS Health to improve health outcomes and connect members to appropriate care. Qualifications include active, unrestricted licenses and board certification; multi-state licenses preferred. Full-time role with comprehensive benefits.
