Job Summary
Care Partner provides in-home dialysis support for a 1:1 client in Norcross, GA, assisting with dialysis treatment setup (NxStage or Tablo), maintaining a clean and safe treatment environment, offering companionship and emotional support, helping manage treatment schedules, providing light ADL assistance, monitoring safety, and documenting shift notes. Training is provided by the clinical team to ensure proficiency with patient-specific dialysis routines and home setup. The role emphasizes compassionate, patient-centered care and collaboration with clinicians, with consistent, client-focused hours (4-5 days/week).
Required Qualifications
- 1+ year of dialysis experience (home or clinic)
- Familiarity with home dialysis systems preferred (NxStage, Tablo is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Steady, supportive, and patient — especially during an adjustment period
- Strong communicator who works well with a clinical team
- Must pass a Herewith background check
