Job Summary
In-Home Dialysis Care Partner (1:1 Client) supports a patient transitioning to home dialysis, performing dialysis setup with the NxStage machine, maintaining a clean and safe treatment environment, offering companionship and light ADL support, ensuring adherence to treatment times, monitoring safety, and providing shift notes to the coordinating nurse. Requires 1+ year of dialysis experience, familiarity with home dialysis systems (NxStage a plus), willingness to complete training, and ability to work independently with clinical team support. Training and ongoing clinical guidance provided; role focuses on patient dignity, stability, and confidence during the home-based treatment transition.
Required Qualifications
- 1+ year of dialysis experience (home or clinic)
- Familiarity with home dialysis systems preferred (NxStage is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Strong communicator who works well with a clinical team
- Must pass a background check
