Job Summary

Provide skilled nursing care in patients' homes, coordinating quality patient care per physician orders while continuously assessing conditions and reporting changes. Document all activities, assessments, and nursing actions in electronic records, develop and update care plans, and implement interventions to prevent or reduce risks. Educate patients and families on disease processes and self-care techniques, ensuring infection control measures protect staff and patients. Maintain knowledge of nursing competencies through continued education and skill evaluations while adhering to state regulations and Aveanna policies.

Required Qualifications

  • Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
  • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Reliable transportation
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current, unrestricted state license as a Registered Nurse in the state of practice
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
  • One (1) year of pediatrics experience (South Carolina specific)
  • One (1) year of experience required working under current nursing license (California specific)
  • One (1) year of experience required working as a licensed nurse (Louisiana specific)
  • Continuing Education as required by state
  • Two (2) years Registered Nurse experience with cardiac monitoring and oncology experience (Medical Oncology RN)
  • Two (2) years experience with vital assessment, plan of care development, and telephone triage (Ambulatory RN)
  • Two (2) years Registered Nurse experience (Medical Surgical RN)
  • Two (2) years of Registered Nurse, preferably with oncology experience (Peri-Operative Care RN)
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
  • Required vaccinations and testing, subject to exemptions for medical or religious reasons

Desired Qualifications

  • Six months prior hands-on nursing experience preferred but not required
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
