Job Summary
Deliver skilled nursing care for low-acuity clients in home settings, managing feeding tubes and medication administration under RN supervision. Continuously assess patient conditions, document all activities and nursing actions in electronic records, and update care plans based on evolving needs. Implement infection control measures and educate patients and families on disease processes and self-care techniques. Recognize changes in patient status requiring intervention to prevent or reduce risk while maintaining professional accountability. Maintain knowledge of nursing competencies through continued education and skill evaluations.
Required Qualifications
- Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Must have reliable transportation
- Must be able to speak, write, read and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to sit and climb stairs
- Must have visual and hearing acuity
- Must have strong sense of smell and touch
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate

Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
