Job Summary

Provide and document skilled nursing care for low-acuity patients in the home, including feeding tube management and medication administration under RN supervision. Continuously assess patient conditions, report changes to the supervisor or physician, and implement updated care plans to achieve expected outcomes. Educate patients and families on disease processes, self-care techniques, and prevention strategies while maintaining strict infection control and confidentiality standards. Ensure timely documentation of all nursing actions and responses in electronic systems. Maintain professional accountability by complying with Aveanna policies, state regulations, and the Nurse Practice Act.

Required Qualifications

Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
Compact licenses must be transferred to your state of residence within 90 days
Current CPR certification (with hands-on component)
TB skin test (current within last 12 months)
Must have reliable transportation
Must be able to speak, write, read and understand English
