Job Summary
Licensed Practical Nurse (LPN/LVN) provides in-home skilled nursing care under the supervision of a Registered Nurse, following the individualized care plan and physician orders. Responsibilities include documenting all nursing activities, observations, and coordination of care; participating in and updating the nursing care plan; recognizing changes in patient condition and implementing appropriate nursing actions; maintaining infection control practices per OSHA standards; and educating patient and family on disease processes, self-care techniques, and prevention. Requires an active LPN/LVN license, CPR certification, TB test, ability to travel, and flexibility to work full-time or part-time schedules, including day and night shifts for pediatric patients. Previous pediatrics experience and home-health experience are preferred.
Required Qualifications
- Active LPN/LVN license in the practice state
- Current CPR certification
- TB skin test within last 12 months
- Six months preferred nursing experience (not required)
- Reliable transportation
- Graduate of an accredited school of nursing
