Job Summary

Deliver and coordinate quality patient care in compliance with physician orders while continuously assessing patient conditions and reporting changes to supervisors. Document all activities, assessments, nursing actions, and care coordination in a timely manner using electronic tablets in each home. Implement and update nursing care plans, taking appropriate actions based on assessments to achieve expected outcomes and prevent risks. Educate patients and families regarding disease processes, self-care techniques, and prevention strategies. Maintain knowledge of nursing competencies through continued education and skill evaluations while adhering to infection control measures and professional boundaries. Provide skilled nursing care for pediatric clients with feeding tubes and medication administration under RN supervision.

Required Qualifications

Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
Compact licenses must be transferred to your state of residence within 90 days
Current CPR certification (with hands-on component)
TB skin test (current within last 12 months)
Must have reliable transportation
Must be able to speak, write, read and understand English
