Job Summary
Licensed Practical Nurse (LPN/LVN) for in-home pediatric/home-health care. Provide skilled nursing care under RN supervision, follow care plans, document nursing actions, monitor patient condition, and educate family. Must maintain a valid state license, CPR certification, TB test current, and have reliable transportation. Flexible scheduling including full-time, part-time, or PRN with day, evening, and weekend shifts; travel to patients’ homes required. Responsibilities include delivering quality patient care, coordinating with the care team, maintaining infection control per OSHA standards, and participating in ongoing education and skill development. Strong documentation, assessment, and communication skills are essential.
Required Qualifications
- Active, unencumbered LPN/LVN license in practicing state
- Graduate of an accredited school of nursing
- Current CPR certification
- TB skin test current within last 12 months
- Six months hands-on nursing experience preferred
- Reliable transportation
