Job Summary
Licensed Practical Nurse (LPN/LVN) for in-home care of adult patients with high acuity, including feeding tube, tracheostomy and ventilator care. Schedule includes weekends (Friday-Sunday, 6pm-6am), with PRN availability and flexible options for full-time, part-time, or PRN. Responsible for delivering and coordinating skilled nursing care under RN supervision, documenting care, updating care plans, ensuring infection control per OSHA standards, and educating patients/families on disease processes and self-care. Requires active nursing license in the practice state, CPR, ability to travel, and readiness to perform clinically and document thoroughly in home settings. Required Qualifications
- Active unencumbered RN license in state of practice
- Current CPR certification
- TB skin test (within last 12 months)
- Six months nursing experience preferred but not required
- Reliable transportation
- Graduate of an accredited school of nursing
- Compact licenses must be transferred within 90 days
- State-specific requirements as applicable (SC, CA, LA)
- Continuing education as required by state
