Job Summary

Deliver skilled nursing care in the home for toddlers with high acuity needs, including feeding tube management, tracheostomy care, ventilator support, and medication administration. Continuously assess patient conditions, document all activities and nursing actions in electronic systems, and implement updated care plans based on physician orders. Recognize changes in patient needs requiring intervention to prevent or reduce risk while educating families on disease processes and self-care techniques. Maintain infection control measures, comply with state regulations and the Nurse Practice Act, and respond to emergency situations in the home environment.

Required Qualifications

  • Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
  • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Reliable transportation
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to sit and climb stairs
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
  • Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
  • Must comply with state specific requirements (South Carolina: One year of pediatrics experience; California: One year of experience required working under current nursing license; Louisiana: One year of experience required working as a licensed nurse)
  • Continuing Education as required by state
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures

Desired Qualifications

  • Six months prior hands-on nursing experience preferred but not required
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
