In Home Healthcare LVN: High Acuity (Weekend Days)

Job Summary
Deliver skilled nursing care to toddler patients in the home, managing high-acuity needs including feeding tubes, tracheostomies, ventilators, and medication administration under RN supervision. Continuously assess patient conditions, document all activities and care plans electronically via tablet, and educate families on disease processes and self-care techniques. Recognize changes in patient needs requiring intervention to prevent or reduce risk while adhering to infection control and OSHA standards. Maintain personal accountability by complying with state regulations and Aveanna policies. Requires an active LPN/LVN license, current CPR certification, and reliable transportation. Schedule includes Friday-Sunday shifts from 8am-8pm in Selma/Schertz, TX.
Required Qualifications
- Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Reliable transportation
- Must be able to speak, write, read and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to sit and climb stairs
- Must have visual and hearing acuity
- Must have strong sense of smell and touch
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
- One (1) year of pediatrics experience (South Carolina)
- One (1) year of experience required working under current nursing license (California)
- One (1) year of experience required working as a licensed nurse (Louisiana)
- Continuing Education as required by state
Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
