In Home Healthcare LVN: High Acuity - Nights (PRN)

Licensed Practical Nurse (LPN/LVN) for in-home care providing and documenting skilled nursing per care plan and physician orders under RN supervision. Performs ongoing patient condition assessments, documents nursing actions and responses, participates in updating the care plan, applies infection-control measures per OSHA, educates patient and family on disease process and self-care, and supports skill advancement through ongoing education; requires active state license, CPR, and ability to travel for home visits; shift options include nights, days, weekends, with PRN scheduling.
