Job Summary
Licensed Practical Nurse (LPN/LVN) in home healthcare providing skilled nursing care under the supervision of a Registered Nurse, following physician orders and care plans. Responsibilities include delivering and coordinating quality patient care, ongoing assessment of patient condition, timely documentation of activities and nursing actions, participation in updating the nursing care plan, infection-control practices per OSHA standards, educating patients and families on disease processes and self-care, and maintaining professional competencies. Requires active LPN/LVN license, CPR certification, TB test within 12 months, and willingness to travel; six months of recent clinical nursing experience preferred; home-health experience a plus.
Required Qualifications
- Graduate of an accredited school of nursing
- Active, unencumbered LPN/LVN license in the practicing state
- Current CPR certification
- TB skin test current within last 12 months
- Six months prior hands-on nursing experience preferred
- Reliable transportation
