Job Summary
Licensed Practical Nurse (LPN/LVN) providing skilled nursing care in patients' homes under the supervision of an RN. Schedule options include full-time or part-time days (8a-4p) in Saint Hedwig, TX. Responsibilities include delivering and documenting nursing care per care plans, observing patient condition, updating the care plan, coordinating care, infection-control per OSHA, educating patient and family, and ensuring compliance with the Nurse Practice Act. Qualifications include active LPN/LVN license, CPR certification, TB test, and state-specific requirements; pediatric/home-health experience preferred. Required Qualifications
- Must have and maintain an active, unencumbered LPN/LVN license in the state of practice
- Current CPR certification
- TB skin test current within last 12 months
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
- Graduate of an accredited school of nursing
- Preferences: Six months of recent experience as a Licensed Nurse in a clinical care setting; Home health experience
- State-specific requirements: South Carolina – pediatrics 1 year; California – 1 year under current license; Louisiana – 1 year under current license; Continuing Education as required by state
- Current, unrestricted state license as an LPN/LVN
- Current CPR certification
- Demonstrated proficiency in clinical assessments and documentation
- Travel ability
- English language proficiency
- Must be able to lift 50 pounds
- Must be able to move patients/equipment
