Job Summary
Licensed Practical/Nurse (LPN/LVN) in-home care for pediatric and adult patients. Provides skilled nursing care under RN supervision per care plan and physician orders; documents activities, assessments, nursing actions, and care coordination; participates in updating the nursing care plan; administers medications and manages care to meet patient needs. Responsibilities include infection control per OSHA standards, patient and family education, and maintaining nursing competencies. Requires active LPN/LVN license in the practicing state, CPR certification, and meeting state-specific requirements; flexible scheduling with full-time or part-time shifts (days, evenings, nights, weekends) and the possibility of PRN. Required Qualifications
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice (LPN/LVN)
- Current CPR certification
- TB skin test (last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliability in transportation
- Home health experience preferred
