Job Summary
Provide skilled nursing care to adult patients in home settings, including tracheostomy and ventilator support with training provided. Monitor patient conditions, document all assessments and interventions, and update care plans under RN supervision. Educate families on disease processes and self-care techniques while maintaining strict infection control and confidentiality standards. Manage care coordination and risk prevention in compliance with state regulations and physician orders. Weekend shifts available Saturday-Sunday; full-time, part-time, or PRN options with flexible scheduling.
Required Qualifications
- Licensed Practical / Vocational Nurse – LPN/LVN
- Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Reliable transportation
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
- Must be able to speak, write, read and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to sit and climb stairs
- Must have visual and hearing acuity
- Must have strong sense of smell and touch
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate
Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
- Attention to detail
- Time Management
- Effective problem-solving and conflict resolution
- Good organization and communication skills
- Must be able to adequately respond physically and mentally to emergency situations in the home or during transport
