Job Summary
Conduct 45-minute in-home health evaluations for health plan members, including medication reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams using an iPad clinical workflow. Provide Diagnostic & Preventive Services products as ordered by the health plan while connecting one-on-one with patients to bridge care gaps. Perform virtual visits as needed and travel locally within licensed states to reach patients in their private residences. Maintain comfort and safety in diverse home environments that may contain pets, smoke, or allergens. This part-time role supports Signify Health's mission to simplify care by visiting millions of people nationwide, offering a per diem base rate plus visit incentives and malpractice coverage.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Able to cover the following primary locations: Staunton and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
