Job Summary
Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad and clinical workflow to connect one-on-one with patients in their private residences, reviewing steps to entry and home conditions to inform care referrals. Perform virtual visits as needed while maintaining flexibility to travel locally within licensed states. Engage directly with patients to bridge care gaps and support their journey toward better health.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- MDs and DOs
- Able to cover the following primary locations: Roanoke, VA
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
