Job Summary
Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and ordered diagnostic tests such as spirometry or diabetic eye exams. Utilize an iPad and clinical workflow for each visit while connecting one-on-one with patients in their private residences. Perform virtual visits as needed and maintain flexibility to travel locally within licensed states. Provide all Signify Health services indicated, including Diagnostic and Preventive Services products. Work part-time with anticipated weekly hours of 29.5, covering New London and surrounding areas. Candidates must be comfortable performing duties in diverse home environments that may contain pets, secondhand smoke, or allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Able to cover the following primary locations: New London and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
