In-Home Health - Physician (Full Time) - Colorado (Traveler)

In-Home Health Physician (Full Time) for Signify Health, a CVS Health company, travels up to 100% across defined coverage areas to perform in-home health evaluations. The visit lasts about 45 minutes and includes medication/history review, physical evaluation, and potential diagnostic tests as ordered by the health plan. Responsibilities include engaging with patients in their homes, using an iPad with clinical workflow, not prescribing or altering current treatment, and coordinating care to connect members with appropriate services. Requires active, unrestricted physician license(s) in coverage area(s) and ability to obtain/maintain multi-state licenses; travel locally and within licensed states; benefits include health/dental/vision insurance, PTO, 401K, CEU allowance, malpractice coverage.
