Job Summary
In-Home Health Evaluations by a Nurse Practitioner or Physician Assistant (Part Time) with Signify Health/CVS Health. The clinician visits members in their homes for 45-minute health evaluations, conducts medication and medical history reviews, physical evaluations, and may perform additional diagnostic tests if ordered by the health plan. The role uses an iPad and a clinical workflow; prescribing, ordering labs, or altering current treatment is not required. Responsibilities include building trusted relationships, visiting members nationwide, providing diagnostic and preventive services, and traveling locally to Wilmar, MN and surrounding areas (and similarly in Evansville, IN per anticipated locations). Virtual visits are possible. Required qualifications include an active unrestricted license in the coverage area, board certification, and multi-state licenses (preferred). This is a part-time position with 40 anticipated weekly hours and a pay range of $41.85–$90.13 hourly, with base compensation reflecting the position’s part-time status.
