Job Summary
Provide in-home health evaluations as part of Signify Health-CVS Health network. Clinicians (Nurse Practitioner or Physician Assistant) conduct 45-minute visits with patients in their homes, including medication and medical history review, physical evaluation, and optional diagnostic tests ordered by health plans (e.g., spirometry, eye exams, bone density). Use an iPad-based clinical workflow; no prescription changes or lab orders required. Travel locally within licensed states; conduct virtual visits as needed. Part-time role with anticipated 40 hours weekly and benefits for part-time staff. Primary work setting is the member's home; flexible, patient-centered care delivery tailored to individual health needs. Requirements include active, unrestricted licenses, board certification, and ability to cover Wilmar and surrounding locations; multi-state licenses preferred.
