Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations in Wilmar, MN. Visits last about 45 minutes and include medication/medical history review, physical evaluation, and, if ordered by the member’s health plan, additional diagnostic tests (e.g., spirometry, eye exams). Use an iPad and a streamlined workflow; not required to prescribe, order labs, or alter current treatment. Responsibilities include connecting with members in their homes, visiting millions nationwide through Signify Health’s network, traveling locally, and conducting virtual visits as needed while delivering Diagnostic & Preventive Services. Primary workplace is the member’s residence; environment may include pets and other allergens. Requires active, unrestricted license(s), board certification, and ability to cover Wilmar and surrounding locations; multi-state licenses preferred. Anticipated weekly hours: 40; Part-time.
