Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in Wilmar, MN, visiting members in their homes. Each visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams) as ordered by the member’s health plan. Role utilizes an iPad and standard clinical workflow; clinicians are not required to prescribe medications, order labs, or alter current treatment. Travel locally within licensed states is required; virtual visits are possible. Primary workplace is the member’s home; environment may include pets or other irritants. Active, unrestricted NP/PA licenses with board certification are required; multi-state licenses are preferred.
