Provide In-Home Health Evaluations as a Part-Time Clinician (Nurse Practitioner or Physician Assistant) in Wilmar, MN, traveling to members' homes to conduct a ~45-minute visit that includes medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams). Use an iPad and clinical workflow; not required to prescribe medications or alter current treatment. Expected to travel locally within licensed states and conduct virtual visits as needed, delivering Diagnostic & Preventive Services in the member's home.
Required Qualifications
