In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Wilmar,MN

Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for Signify Health under CVS Health. Visit members in their homes for approximately 45 minutes, including medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, bone density screening, diabetic eye exams). Use an iPad and clinical workflow; no prescribing changes or order modifications required. Responsibilities include engaging with members to improve health outcomes, traveling locally within licensed states, and conducting virtual visits as needed; primary workplace is the member's home with potential exposure to pets and allergens. Requires active, unrestricted licensure, board certification, and ability to cover Wilmar and surrounding locations; multi-state licenses preferred.
