Job Summary
In-Home Health Clinician (Nurse Practitioner or Physician Assistant) (Part Time) providing in-home health evaluations to patients in Wilmar, MN and surrounding areas. Visits last about 45 minutes and include medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by the health plan. Clinicians use an iPad and a clinical workflow; no prescribing, lab orders, or treatment changes are required. Flexible to travel locally, conduct virtual visits as needed, and perform Diagnostic & Preventive Services in the home setting. Primary workplace is the patient’s residence, which may include pets or allergens. Encourages building trusted relationships to improve health outcomes and connect patients to appropriate care. Requires active, unrestricted licensure, board certification, and (preferably) multi-state licenses; primary location Wilmar and surrounding areas. Part-time position with selective benefits; anticipated weekly hours around 40; hourly pay range $41.85-$90.13 depending on experience and geography.
