Job Summary
In-Home Health Clinician role for Nurse Practitioner or Physician Assistant delivering in-home health evaluations. Conduct 45-minute visits with one-on-one engagement, including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as directed by health plans. Use an iPad with a streamlined clinical workflow; no prescribing, ordering labs, or changing current treatments required. Travel locally to member homes, with potential for virtual visits, providing Diagnostic & Preventive Services and connecting members to appropriate care while partnering with Signify Health/CVS Health network. Part-time position with benefits including malpractice insurance and a comprehensive benefits package; requires active licenses and board certification, with multi-state licenses preferred.
