Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations in patients' homes. Visit lasts ~45 minutes, including medication and medical history review, physical evaluation, and potential diagnostic tests (e.g., spirometry, bone-density screening) as ordered by the health plan. Use an iPad and clinical workflow; not required to prescribe meds, order labs, or alter current treatment. Travel locally within licensed states; conduct virtual visits as needed. Primary workplace is the member's home, which may include pets or allergens. Active NP/PA license with board certification and ability to travel; role supports Signify Health services and Diagnostic & Preventive Services.
