In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Wilmar,MN

In-Home Health Clinician on a Part-Time basis (Nurse Practitioner or Physician Assistant) for Signify Health, part of CVS Health. Conduct in-home health evaluations for members, including 45-minute visits with medication and medical history review, physical evaluation, and potential diagnostic tests as ordered by the member’s health plan. Use an iPad and a clinical workflow; not required to prescribe meds or alter treatment. Responsibilities include visiting members at home, building trusted relationships, coordinating with the health network, and traveling locally within licensed states. Primary work in members' homes with environments that may include pets and allergens. Anticipated weekly hours are 40. Active NP/PA license(s) with board certification; multi-state licensing preferred. Primary locations Wilmar, MN and surrounding areas; and Tacoma, WA and surrounding locations. Part-time clinicians qualify for select benefits, malpractice coverage during Signify Health activities, and supplies.
