Job Summary
Part-time Nurse Practitioner or Physician Assistant to perform in-home health evaluations for CVS/Signify Health members. Role involves one-on-one visits (~45 minutes) in members' homes, reviewing medications and medical history, conducting physical evaluations, and performing additional diagnostic tests if ordered by the health plan. Clinicians will use an iPad-based workflow, travel locally to member homes, and may conduct virtual visits as needed. No prescribing or treatment plan changes are required during visits. Requires active, unrestricted licenses, board certification, and ability to cover Wilmar and surrounding areas; multi-state licenses preferred. This role emphasizes building trusted relationships and connecting members to appropriate care in a home-based setting.
