Job Summary
In-Home Health Evaluations for Nurse Practitioner or Physician Assistant (Part Time) with Signify Health at CVS Health. Conduct 45-minute visits in members’ homes, review medication history, perform physical evaluations, and, if ordered, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Use an iPad and a clinical workflow to deliver personalized, at-home health assessments, connect members to the right care, and support health-plan goals. Travel locally within licensed states as needed and conduct virtual visits as required. Primary workplace is the member’s residence; environment may include pets or household triggers. Requires active NP or PA license, board certification, and ability to cover Wilmar and surrounding locations; part-time role with anticipated weekly hours of 40 and pay range $41.85–$90.13 per hour.
