Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for Signify Health, delivering 45-minute visits at members' homes. Responsibilities include conducting medication and medical history reviews, physical evaluations, and ordering/performing diagnostic tests as indicated by health plans (e.g., spirometry, bone density screening) using an iPad workflow. No prescribing or altering treatment plans required. Flexible to travel locally within licensed states; virtual visits as needed. Primary workplace is members' homes with potential exposure to pets and household allergens.
