In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Wilmar,MN

In-home health clinician role for Nurse Practitioner or Physician Assistant (Part Time) delivering in-home health evaluations to patients in their homes. The visit lasts about 45 minutes and includes health questions, medical history review, physical evaluation, and, if ordered by the patient’s health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Clinicians will use a mobile iPad-based workflow and are not required to prescribe medications, order labs, or alter current treatment regimens. Responsibilities include building trusted relationships, providing flexible, comprehensive, and personalized health evaluations, traveling locally to member homes, conducting virtual visits as needed, and delivering diagnostic and preventive services as indicated. Primary locations include Wilmar and surrounding areas; work is in the member’s home environment with potential allergens. Requires active license, board certification, and ability to cover the specified service area; multi-state licenses preferred.
