Job Summary
In-Home Health Evaluations by a Nurse Practitioner or Physician Assistant providing 45-minute visits in members' homes. Role includes medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams). Use of an iPad with a clinical workflow; not required to prescribe meds, order labs, or alter current treatment. Travel locally to member homes within licensed states; provide virtual visits as needed; primary workplace is the member's residence and may include environments with pets or smoke.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover Wilmar and surrounding locations
