Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in Wilmar, MN. Visits last about 45 minutes, including medical history review, physical evaluation, and patient education; may include additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening) if ordered by the health plan. Use of an iPad and a clinical workflow; not required to prescribe medicines, order labs, or alter current treatment.
