Job Summary
In-Home Health Clinician role providing in-home health evaluations for Signify Health (Part Time). Clinician responsibilities include conducting 1:1 visits lasting ~45 minutes, reviewing medication and medical history, performing physical evaluations, and, if ordered by the health plan, arranging additional diagnostic tests (e.g., spirometry, diabetic eye exams). Use of an iPad with a clinical workflow; no prescribing, ordering labs, or altering current treatment during visits. Travel locally within licensed states; virtual visits as needed; primary workplace is the patient’s home with potential exposure to pets and household allergens. Requires NP or PA credentials, board certification, and active/unrestricted licensure; ability to cover Wilmar and surrounding locations; part-time weekly hours around 40; competitive benefits and compensation available.
