Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for Signify Health across Wilmar, MN and surrounding areas. Each visit lasts about 45 minutes and includes a medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests. Use an iPad and a streamlined clinical workflow; no requirement to prescribe medications, order labs, or alter existing treatments. Responsibilities include building trusted relationships, visiting millions of people nationwide through Signify Health, traveling locally as needed, and conducting virtual visits when required.
